Breast Implant Types

Saline Implants

Saline implants are a silicone elastomer shell that is filled with a saltwater solution known as saline. This is the same solution that patients get when having an IV. Saline implants come deflated and are filled with the saline once inserted into the breast. Saline implants have a slightly higher tendency to ripple than cohesive gel implants and therefore need to be placed under the muscle more often. Saline implants tend to provide a more rounded appearance to the breast.

Cohesive Gel Implants

A cohesive gel breast implant is a silicone shell filled with a silicone gel inside. These implants, which have been available for breast augmentation since the year 2000, differ greatly from the previous generation of silicone implants. To understand the consistency of a cohesive gel implant, many patients are told to bite into a gummy bear candy and look inside. For this reason, many patients ask their doctors for “gummy bear” implants rather than referring to them as cohesive gel implants. These implants are widely recognized for their ability to closely mimic the soft, supple appearance and feel of natural female breasts. A cohesive gel implant is filled with a thicker substance than liquid silicone and does not leak if the implant shell holding it inside the breast is ruptured. Cohesive gel implants come in two shapes, an anatomical, and a round shape. The beauty of the anatomical cohesive gel implants is that they come in a variety of heights, widths, and projections, while with a round implant you can only alter the width and projection. It should be noted that anatomical cohesive gel implants are best for women seeking natural-looking breasts that are roughly proportionate to their body structures. Anatomical cohesive gel implants are not recommended for women who are seeking overly large breast enhancement. Round Versus Anatomical

Breast Implant Shapes
Breast implants are available in both round and contoured shapes. The most common type of implant is the round one. Contoured implants provide for teardrop shaped breasts and can offer various amounts of projection. In general, women with narrow chest walls require implants that offer greater projection. During the consultations, Dr. S. Rice takes the time to understand the reasons the patient has opted for breast augmentation as well as the size and breast shape she wants. Dr. S. Rice then suggests the appropriate implant shape to provide the patient with the results she desires. Dr. S. Rice understands that when it comes to breast augmentation, each patient has her own reasons and expected results, and works to ensure that the patient gets the breasts that she wants.
Breast Implant Sizes
Size is, of course, a major consideration when it comes to breast augmentation. By and large, the objective of breast implants is to achieve a fuller bust that is also in proportion to the overall physique. In general, an increase of up to two cup sizes is the norm, but the specifics vary from case to case. The size of the implants and the effect that they will achieve is part of the detailed consultations and discussions that Dr. S. Rice has with all breast implant patients. Dr. S. Breast Augmentation FAQ's
Breast enlargement, also called breast augmentation, is a surgical procedure designed to increase the size of the breasts. In breast enlargement surgery, breast implants filled with silicone cohesive gel or saline are placed under the skin of the chest. The breast implant is placed either in front of or behind the pectoral muscle, making the breasts fuller. After making incisions in the skin, typically in the armpit (trans axillary) or the breast itself — either around the pink area surrounding the nipple (periareolar) or in the natural crease under the breast (inframammary fold), Toronto plastic surgeon Dr. Rice inserts the breast implants and moves them into aesthetically pleasing positions. Breast enlargement can be combined with abreast lift procedure to firm up and lift sagging breasts more dramatically.
Next to choosing a size of breast implant I believe the next decision that women struggle with is which incision should I chose to insert the breast implant. The three most common incisions to insert the breast implant in Toronto are: thru the natural crease under the breast ( inframammary fold or IMF); thru the areola or the pink area around the nipple ( periareolar ) or thru the armpit ( trans axillary). What we have found is that most women base their decision on what their girlfriend had or what they believe to be the best incision to hide the scar. Generally, all incisions are about the same size – somewhere around 4 cm, so the goal is where can YOU hide that scar the best. Dr. Rice and his staff will guide you thru the PRO and CONS of each incision as it applies to you specifically, remember your breasts are never the same as your girlfriends or a picture you saw on the internet, so what works well for them may not work well for you. Remember this is surgery and there will be a scar afterward, and all women scar differently.

Breast augmentation thru the inframammary fold (IMF) or crease under the breasts

The natural crease under a women’s breast is known as the inframammary fold. This incision is generally the most popular incision to use when inserting a breast implant. The resultant scar is generally hidden under the breast. For women choosing an anatomical or tear drop shaped breast implant, this is generally the only incision you can use, as it is the only incision that the surgeon can assure that the breast implant is straight. The inframammary fold incision is generally the incision with the least complications.

Breast augmentation using the periareolar or around the nipple approach

The areola is the darkened circle that surrounds the nipple. If you think of the areola as a clock this incision goes along the bottom of the circle from the 3 o’clock to the 9 o’clock position. The first thing to realize is that your areola must be large enough to accommodate the implant, so for women with very small areola, this incision may not work. In order to place the implant either above or below the muscle the dissection must go thru the centre part of the breast, this is generally where the milk ducts and nerve endings to the nipple are found. For younger women who have not breastfed yet, they need to realize that this approach may interfere with breastfeeding in the future.

Breast augmentation using the trans axillary or thru the armpit approach

Using the trans-axillary approach the incision is not on the breast but rather thru the armpit. Generally, a round implant is needed using this approach. It can be difficult to insert a cohesive gel or gummy bear implant thru this incision and is usually best for a saline implant. Even though there is no scar on the breast there will be a scar in the armpit and this may be an issue when wearing certain types of clothing that expose the armpit.

Recovery from breast augmentation is quite variable between individuals, some people react to surgery better than others. You need to remember that the recovery phase is just crucial as the surgery itself and trying to speed up your recovery may lead to negative aesthetic results. Generally, you want to take it easy for the first few days, nothing that is going to raise your blood pressure too high and cause bleeding or an incision to open. There is a slight difference in the recovery phase depending on whether the implant is placed above or below the muscle, with below the muscle a little more uncomfortable postoperatively. Generally, most people are back to life like normal within one to two weeks after surgery. The time it will take to recover from breast augmentation surgery depends on several factors, including the type of procedure used to place the breast implants, the overall physical condition of the patient, and other factors. However, you can expect to return to work and most daily activities within a week of the surgery, while exercise and other vigorous physical activities can typically be resumed after two to four weeks.

As with all surgeries, breast augmentation enlargement surgery may result in some side effects. Infection, bleeding, numbness of the nipples and shifting, hardening, or rupture of the implant may occur, although the vast majority of breast enlargement surgeries performed are successful, without any complications or side effects. Breast augmentation surgery does have risks.  Common to any surgery is the risk of infection and bleeding after surgery.  Specific to breast augmentation there is a risk of losing sensation to the nipples or small areas of the breast.  For those women who have not breastfed there may be a risk of interfering with breastfeeding in the future, however, we have had lots of patients who have breastfed after breast augmentation.  There is a risk that the implant may shift in position, this is usually due to a capsular contracture or thinking of the scar tissue that may build up around the implant. A secondary surgical procedure may be necessary to address a capsular contracture.  There will come a point in time that the implant will need to be replaced. Occasionally due to a combination of the type of implant and thin skin a woman can develop rippling of the implant.  The vast majority of breast enlargement surgeries performed are successful, without any complications or side effects
